Funding Priorities:
- Initiatives that improve population health, ensure access to quality care, healthy and affordable food, and empowers Naugatuck residents to actively participate in managing their health.
- Efforts to expand and connect community resources to work toward a common vision of health that addresses the unique needs of Naugatuck.
- Promote collaboration from multiple sectors to improve conditions and outcomes related to health and well-being.
- Community awareness and increased dissemination of evidence-based prevention and chronic disease management programs (ex. Cancer, diabetes, obesity, tobacco use, asthma, falls).
- Identification of barriers to implement effective prevention models and development of strategies to address these barriers (e.g. medical transportation, affordability, responsive care)

First Time Applicants:
The Foundation welcomes new applicants. We want to get to know our grantees and learn more about your work. In advance of submitting a first-time request to the Foundation, applicants are required to set up an in-person meeting with the Program Officer who staffs the area of interest of the request. These conversations are an important part of the process and are tied to eligibility. Please contact the program officer to set up a meeting as soon as possible.
